A third-round pick out of a Tallahassee, Fla., high school in 2014, left fielder Matt Railey hit just .165 with zero extra-base hits in 29 games this season, most of them at low Class A Kane County. That performance didn’t cut it for a bat-first player like Railey, who also weathered a 50-game suspension in 2015 for failing a test for amphetamines.

The Reds’ Alfredo Simon has run up a 9.45 ERA this season, giving him the highest mark for any major league pitcher with at least 10 starts. In a strange twist of fate, the next two starters on the list were traded for one another in the offseason: Braves rookie Aaron Blair (7.99 ERA in 11 stats) and Diamondbacks intended No. 2 starter Shelby Miller (7.14 ERA in 14 starts). Note that Atlanta also acquired Dansby Swanson and Ender Inciarte when they traded Miller to Arizona.

While Blair struggled initially at Triple-A Gwinnett, he had righted the ship in three starts leading up to his placement on the disabled list last week. He went 1-0, 2.25 with 19 strikeouts in 20 innings immediately before he tweaked his left knee in a side session.

The Orioles signed 31-year-old reliever Logan Ondrusek to a major league deal after he spent the past two seasons in Japan, recording a 2.17 ERA in 102 appearances for Yakult of the Central League. He struck out 8.2 and walked 3.0 per nine innings in Japan, while allowing just 75 hits in 100 frames. Ondrusek last pitched in the U.S. majors for the 2014 Reds.

One of the more snakebit prospects in baseball, low Class A West Michigan center fielder Derek Hill needs Tommy John surgery and will be out until at least midseason 2017. This will be the fifth trip to the disabled list in the past two seasons for Hill, a first-round pick out of high school in 2014. He hit .266/.312/.349 with a Midwest League-leading 35 stolen bases and 66 runs scored this season.

Triple-A Las Vegas third baseman T.J. Rivera led the Pacific Coast League with a .349 average at the time the Mets called him up, and, in fact, he leads all active players (min. 2,500 plate appearances) with a .323 career average in the minor leagues. The 27-year-old infielder signed as a nondrafted free agent out of Troy in 2011, though he attended high school in the Bronx.

Said one Mets official about Rivera last offseason: “He is one of those guys who has a knack for barreling ball. He can play all over the infield.”

The No. 11 overall pick by the Mariners in June, center fielder Kyle Lewis had surgery on Aug. 11 to repair a torn anterior cruciate ligament and the meniscus in his right knee. The short-season Everett star injured his knee on July 19 when he collided with the opposing catcher, and Seattle is now officially transferring him to the 60-day disabled list. The BA College Player of the Year this season, Lewis hit .299/.385/.530 with 16 extra-base hits in 30 Northwest League games.

Shoulder trouble is the last thing the Blue Jays want to hear associated with 2014 first-round catcher Max Pentecost, who has had two surgeries on his right shoulder and missed the entire 2015 season. In this case, though, the 23-year-old injured his other shoulder in an awkward slide into home plate, and he went on the low Class A Lansing disabled list. Pentecost, who has played all 62 games this season at DH, hit .314/.375/.490 with seven home runs through 62 games.

The Blue Jays intend to develop trade-deadline pickup Harold Ramirez as a center fielder, but those plans are on hold after the 21-year-old suffered a knee injury and went on the disabled list at Double-A New Hampshire.
